The College Security Authority Incident Reporting Form generates a report that is normally reviewed during business hours and should not be used if you need an immediate response. The link and report form is intended for the sole use of those persons designated as campus security authorities (CSA) at and for Empire State College as required under the Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act (Clery Act) codified as 20 USC 1092 (f) as a part of the Higher Education Act of 1965. All reports made through this form will be confirmed by contacting the campus security authorities reporting the incident.
The college designates certain individuals as campus security authorities.
This link and report form is to be used when a CSA has received information regarding an incident that he or she believes to be a reportable offense under the Clery Act, and when the victim does not wish to report the incident to the police. Please complete all of the fields to provide the college with the best opportunity to respond to this incident.
