Welcome to the Forms homepage. The information provided below is designed to provide access to commonly used Office of Collegewide Disability Services forms for faculty, staff and students.
Please Note: This page does not reflect, nor is it intended to be, a repository of all forms necessary, which students, with or without a disability, may be required to complete as an Empire State College student.
All students with disabilities requesting services or accommodations from the office must complete the Disability Declaration and Request for Accommodations Form upon their first contact with the office.
The forms listed below are available in both Microsoft Word format and PDF format. You do not need to have Word installed on your computer to view the Word documents.
